Greetings! Below is the information on the Textbook Agreement Form you should have had signed by Tuesday, Sept 14th. It is REQUIRED that your book is covered (with a paper bag) and/or in a 1-inch binder.

STUDENT TEXTBOOK AGREEMENT
College Preparatory Mathematics Algebra I Connections Volume 1
(Value $18 each, or $35 for both)
I know that I am responsible for:
• Entering my name inside the back cover of the textbook.
• Keeping the textbook protected with a book cover.
• Bring my textbook to school everyday.
• Reimbursing the school for any damage to and/or loss of textbooks issued to me. (Any misuse of the textbooks due to carelessness or neglect may be considered cause to charge the student a fine for that textbook.)
• Returning the textbook at the end of the semester, or if withdraw from Impact Academy. (Report Cards & Transcripts will not be issued until all books are returned)
Fines:
• Torn Pages 25% cost of book
• Ink or Pencil Marks
o Minor Two dollars per a mark
o Major 25% cost of book
• Loose Binding 50% cost of book
• Minor Water Damage 50% cost of book
• Obscenities Written or Drawn 100% cost of book
• Damage That Prevents Reissuing 100% cost of book
• Removal or Damaging of Book Label 25% to 100% of book
